Services Offered:
Person Centered Planning – a facilitated planning process designed to identify strengths, challenges and barriers to a safe and secure future and then establish an action plan with appropriate services and supports to successfully accomplish the goals.
Form Completion – assistance for individuals and families completing forms for access to services, supports and resources such as ACSD, SSAH, ODSP and Passports.
